CANS

American Society of Addiction Medicine (ASAM) criteria are integrated into Qualifacts’ systems to provide a holistic, biopsychosocial assessment utilized for service and treatment planning as well as level of care determination. Our ASAM assessment integration can be used to determine the appropriate level of care for covered Substance Use Disorder (SUD) treatment services.
PHQ9
The Patient Health Questionnaire (PHQ-9) is a multi-purpose tool utilized for the screening, diagnosing, and monitoring of depression severity. It can be used to assist the clinician in making the diagnosis of depression and to quantify depression symptoms and monitor severity. Qualifacts easily integrates PHQ-9s into our systems to streamline the form collection process.
DLA-20
The Daily Living Activities 20 (DLA-20) is a clinical outcome measure designed to assess current behavior in 20 activities of daily living. Qualifacts has integrated DLA-20 to provide valuable data that drives service delivery and tracks outcomes.
